Why Must I Fill In All These Forms?

When you attend a Microsuction Ear Wax Removal appointment with us, there are two forms that need to be completed: A Triage Form, which everyone must complete before booking, and a Consent Form, which everyone must complete before attending their appointment.

The Triage Form is a new requirement introduced by our professional bodies since Covid-19 that everyone must be triaged before booking a face-to-face appointment, and a record must be kept of this. It is primarily there to ensure that everyone attending an appointment is free of any symptoms of Coronavirus. Because we screen everyone before booking, it makes it safer for everyone. If you do find a local clinic that doesn’t triage everyone, bear in mind that although it may save a minute of your time, the person attending the clinic before you may have Coronavirus symptoms.

The Consent Form, on the other hand, is required to ensure that the patient attending the appointment fully understands the procedure and what is required of them to ensure their safety. Before Covid-19 we used to complete the Consent Form during the face-to-face appointment, but it is now far safer to minimise face-to-face time and completely avoid the sharing or pens, clipboards and even iPads or mobile phones. We have implemented and tested the technology to enable this to be done in the comfort of your home, and have installed security measures, including password protection, firewalls and military grade encryption to ensure the safety of your personal information.
